Example of ACH Fraud Prevention Procedures

Below is an example of how fictional company, ABC Company, has their ACH fraud prevention procedures written.

  1. Dual Authorization – All payment requests must be approved by two authorized employees before release.
  2. Callback Verification – Any request received by email, text, or phone for changes to account numbers, payment amounts, or beneficiary details requires a callback to a pre‑verified phone number.
  3. Restricted Email Instructions – The team will not process payment instructions that come solely from personal email addresses or unverified senders.
  4. Daily Review – A designated team member reviews all outgoing ACH and wire activity at the end of each business day.
  5. Internal Escalation – Any suspicious or unusual request is escalated to the Finance Manager before action is taken.
